Am I eligible?

To find out if you are eligible you must contact My Aged Care who will organise an assessment through the Aged Care Assessment Team (ACAT). If you are eligible for a package, you will receive specified funds/resources and, together with the assistance of a dedicated Care Advisor, you can select and purchase services to best meet your care needs or wishes.

Can I get a home care package privately?

Any self-funded person needing coordinated support services can also purchase Home Care Packages privately. To find out more about private services we offer in your area contact us on 02 9718 6199.

What is the difference between a Home Care Package (HCP) and a Commonwealth Home Support Programme (CHSP)?

Home Care Packages

  • Your eligibility is assessed by the Aged Care Assessment Team (ACAT).
  • Caters to Australians with higher-level care needs.
  • A Case Manager tailors a wholistic plan to meet your higher level and more complex care needs.
  • Packages provide ongoing, wholistic services to support independence at home. Care needs are reviewed regularly to ensure the appropriate services and level of care are in place at all times.

Commonwealth Home Support Programme

  • Your eligibility is assessed by the Regional Assessment Service (RAS) and your specific services are identified.
  • Caters to Australians with low-level care needs.
  • Our staff will develop a plan with you to meet your specific service needs.
  • You can stop and start services according to your changing needs, only contributing towards the services you use.
